So, lets say the empty PC boxcar is at Erie Avenue Yard to be sent home.

Lets use the new 1975 Ticket.

DESTINATION would be Belmont (PC Interchange track)
Route: Erie Avenue Yard, Fairhill Jct, Nicetown, Belmont
Probably just Belmont would be written?

If it were needed for a load (no other Reading cars available) at Company XYZ at Third and Berks Sts.

DESTINATION would be Third and Berks St Station
Route: Erie Avenue Yard, Fairhill Jct, Third and Berks Sts Station
Consigned to: Company XYZ

Just in case you want to see what these documents looked like in years past, here are two 1890's versions. There was a slightly smaller, earlier version also. They were called "car tickets" and they were mentioned in the 1894 rule book with regard to how the conductor had to handle them.
